JRR: Because, the doctors of such patients have to be extra careful, since these patients are educated, and have derived added smartness from net-videos on health issues; here their doctors have to develop special communication skills to convey to their patients whether or not stop medicines; what are the fallacies of these videos, etc., so the doctors have to be more vigilant, and so on...
JRR: Here, 'sick' also can be taken to mean that they get bored and annoyed with such patients, who pose that they are smarter than the doctors!
